Norberry Dental Centre Patient Information
On your first visit, we take a complete medical and dental history and perform a thorough oral examination with the necessary dental x-rays. Your dentist will assess this information and advise you as to the best course of action based on your situation, offer alternatives, and provide you with the information you require to make an educated decision about your dental health.
We reserve your appointment time for you alone. If you are unable to keep a previously scheduled appointment we request a minimum of 2 business days notice of cancellation in order to avoid any charges to your account.
New Patients at Norberry Dental
If you are a new patient to Norberry Dental, please bring with you:
- A Full List of Any Current Medication: Some medications taken for medical reasons impact our dental treatment, so it is important that we are made aware. As well, for patients who have undergone joint replacement surgery or heart valve replacement, it is important to advise the dentist since antibiotics are necessary prior to undergoing most dental procedures.
- Any X-Rays Taken at a Previous Office in the Past 2 Years: X-rays are an important diagnostic tool for our examination. We are happy to reuse current existing x-rays to save you time and money. However, if these are unavailable to us at your first visit or need to be updated to ensure an accurate diagnosis, we may need to take a selection of x-rays so that we can perform our job properly.
- Insurance Information (if eligible): Please supply the name of the insurance carrier, plus any group, policy, ID or contract number applicable to your coverage so that we may better serve you.
- A copy of the Patient Registration Form
Financial and Insurance
We will gladly submit any insurance forms for planned or completed treatment to the insurance company on your behalf. Find out more information here.
